If you want to work towards any of the outdoor qualifications, you need to register your details by completing a registration form when you start to work towards one of the Outdoor Qualifications listed below. You need to allow at least 6 months to complete the qualifications (longer for some of the more specialist ones).
Please complete the form below and make three copies – keep one for your records and give or send:
• One to your District Commissioner.
• One to your Division Residential Adviser.

Once registered you will be invited to an outdoor mentor/mentee workshop to meet with a mentor, who will guide you through what you need to do to gain your qualification on a one to one basis.
This form is for all participants at a residential event. This includes Senior Section members aged under 18yrs and adults taking part in an event where under-18s are involved. Participants aged 16yrs or over may complete and sign their own forms.
Form G/H comes in one part.
The Guider or activity / event organiser:
- completes the event details, including the deadline date
- signs the form
- sends it to the parent / guardian or participant as appropriate
The parent / guardian or participant:
- completes the form
- signs the form
- returns it to the Guider or activity / event organiser by the deadline
This form is used to gain parental consent for each member aged under 18 to take part in an event lasting one night or more. This includes a Senior Secion member under 18yrs attending as part of the leader team.
Form G/C comes in one part.
The Guider or activity / event organiser:
- completes the top half of the form, including the deadline date in the top right-hand corner
- signs the form
- sends it to the parent / guardian
The parent / guardian:
- completes the bottom half
- signs it
- returns the bottom half to the Guider or activity / event organiser by the deadline
This forms is used to gain parental consent for each member aged under 18yrs to take part in an event or activity away form the usual meeting place or at a different time: this includes a Senior Section member aged under 18yrs attending as part of the leader team.
Form G/O comes in one part.
The Guider or activity / event organiser:
- completes the top half of the form, including the deadline date in the bottom half
- signs the form
- sends it to the parent / guardian
The parent / guardian:
- completes the bottom half
- signs it
- returns the bottom half to the Guider or activity / event organiser by the deadline
Application to run a camp / holiday (A/A)
Application for a Guide or Senior Section permit holder to run a camp / holiday (A/P)
Notification of a short camp / holiday or sleepover (A/S)
Notification of a mobile expedition (A/M)
Application to run a boating holiday (A/B)
Application to join a holiday or camp (A/J)
Camp or holiday report form (A/R)
This application form is for all overnight events, camps and holidays in the UK and BGIFC when the number of particpants is equivilant to a unit and it is also used (after gaining permission to plan on form G/P) for:
- events involving travelling abroad
- an internation camp in the UK
- a large holiday or camp (check The Guiding Manual for information about large events)
- a residential event involving members of another organisation
For unit residentials, this form must be completed at least 6 weeks before the event starts
For all other events, this form must be completed at least 10 weeks before the event starts
It may also be used in conjunction with forms A/B, A/J and A/M
Form A/A comes in 4 parts:
top copy: white
second copy: blue
third copy: green
bottom copy: yellow
There is a one-part confirmation slip A/AC attached to the top (white) copy.
The Guider-in-charge:
- completes and signs the form
- keeps the bottom (yellow) copy
- forwards the rest to the appropriate home Commissioner e.g your Disctict Commissioner
The appropriate home Commissioner:
- signs the form
- keeps the third (green) copy
- forwards the rest to the home Camp or Holiday Advisor as appropriate e.g. your Division Brownie or Guide Residential Advisor
The home Camp or Holiday Advisor:
- signs the form
- keeps the second (blue) copy
- forwrds the top (white) copy to the appropriate Advisor. Usually this will be the host Camp or Holiday Advisor i.e the Brownie or Guide Residential Advisor for the Divison in which you will be staying (see the grey booklet "Supporting Information addresses, booklist, forms and index pg 28 for more infomation)
The host Camp or Holiday Advisor:
- completes and signs the confirmation slip
- keeps the top (white) copy
- returns the A/AC slip to the Guider-in-charge
This form is used by a Guide or Senior Section member who wants to run a holiday and is working towards, or has gained, the relevant permit.
Form A/P comes in 5 parts:
top copy: white
second copy: blue
third copy: green
fourth copy: yellow
bottom copy: pink
There is a one-part confirmation slip A/PC attached to the top (white) copy.
The permit holder:
- completes and signs the form
- keeps the bottom (pink) copy
- forwards the rest to the appropriate Guider
The Guider:
- signs the form
- keeps the fourth (yellow) copy
- forwards the rest to the appropriate home Commissioner e.g your Disctict Commissioner
If a Young Leader is taking a group of Guides away, the Guide Guider must also sign the form and may take a photocopy for her own records.
The appropriate home Commissioner:
- signs the form
- keeps the third (green) copy
- forwards the rest to the home Camp or Holiday Advisor as appropriate e.g. your Division Brownie or Guide residential Advisor
The home Camp or Holiday Advisor:
- signs the form
- keeps the second (blue) copy
- forwrds the top (white) copy to the host Camp or Holiday Advisor i.e. the Brownie or Guide Residential Advisor for the Divison in which you will be staying
The host Camp or Holiday Advisor:
- completes and signs the confirmation slip A/PC
- keeps the top (white) copy
- returns the A/PC slip to the permit holder

This form is used by a licence-holder for overnight or short events for Guides or Senior Section members when taking members of her own unit. It is used to notify local Commissioners and Advisors that a sleepover, overnight event or weekend residential event is taking place (lasting up to 3 nights).
Form A/S comes in four parts:
top copy: white
second copy: blue
third copy: green
bottom copy: yellow
The Guider-in-charge:
- completes and signs the form
- forwards the top (white) copy to the host camp or Holiday advisor for information e.g. the Brownie or Guide Residential Advisor for the Division in which you are staying
- forwards the second (blue) copy to the home Camp or Holiday Advisor for information e.g. your Division Brownie or Guide Residential Advisor
- forwards the third (green) copy to the home Commissioner for information e.g. your District Commissioner
- keeps the bottom (yellow) copy
This form is used to gain permission to plan:
a camp or holiday involving travelling abroad
- an international event in the UK (between 9 and 18 months before the event and prior to the planning)
- a large holiday or camp (check The Guiding Manual for information about large events)
- a residential event involving members of another organisation

This form is used to notify the appropriate Commissioners and Advisors that a mobile expedition is being planned. It is used in conjunction with form A/A or A/P.
Form A/S comes in four parts:
top copy: white

The group leader:
- completes and signs the form
- forwards the top (white) copy to the host camp or Holiday advisor for information
- forwards the second (blue) copy to the home Camp or Holiday Advisor for information
- forwards the third (green) copy to the home Commissioner for information
- keeps the bottom (yellow) copy
This form must be signed by the group leader. The approvals of the appropriate Commissioner(s) and Advisor(s) are given on form A/A or A/P.
This form is used to gain permission to run a boating holiday. It should be used in conjunction with form A/A or A/P at least 6 weeks before the event is planned to start.
Form A/B comes in five parts:
top copy: white
second copy: pink
third copy: blue
fourth copy: green
bottom copy: yellow
There is a one-part confirmation slip A/BC attached to the top (white) copy.
The Guider-in-charge:
- completes and signs this form and form A/A or A/P
- keeps the bottom (yellow) copy of both forms
- forwards the rest to the home Commissioner
The home Commissioner:
- signs both forms
- keeps the green copy of both forms (fourth copy of form A/B and third copy of form A/A)
- forwards the rest to the home Camp or Holiday Advisor
The home Camp or Holiday Advisor:
- signs both forms
- keeps the blue copy of both forms (third copy of form A/B and second copy of form A/A)
- forwards the rest to the host Boating Advisor
The host Boating Advisor:
- signs confirmation slip A/BC
- keeps the top (white) copy of form A/B
- forwards the second/pink copy of A/B, confirmation slip A/BC and the top (white) copy of form A/A with the A/AC to the host Camp or Holiday Advisor
The host Camp or Holiday Advisor:
- signs confirmation slip A/AC
- keeps the second (pink) copy of form A/B
- keeps the top (white) copy of form A/A
- forwards the confirmation slips A/BC and A/AC to the Guider-in-charge.

This form is used to gain permission for Brownies, Guiders and Senior Section members to take part in a holiday or camp being organised by another unit outside their District. This should be used in conjunction with forms A/A or A/P at least 6 weeks before the event.

The Guider of the unit applying to join the event:
- completes and signs the form
- keeps the bottomw (yellow) copy
- forwards the rest to her home Commissioner
The home Commissioner:
- signs the top (white) copy
- forwards both parts to the Guider-in-charge of the event
The Guider-in-charge of the event:
- keeps the second (green) copy
- forwards the top (white) copy with form A/A or A/P to her home Commissioner
The home Commissioner:
- keeps the top (white) copy

This form is used to write a report of a residential event by the Camp and Holiday Scheme Assessor after visiting a candidate working towards a residential qualification.
